I have made small little strides that has really helped...
I do not include anything with sugar for an actual meal. I have started measuring my sugar laden crap creamer for my coffee so that I can step down my usage without losing my mind. I do not stock my kitchen with items that I don't want to eat. This means that I have to physically leave the house and go to the store when I have a sugar craving.
So far it has really helped me reduce the amount of sugar that I eat. That final step is going to be the hard one.
Ironically enough, I bake cupcakes and cake pops as a side job and I can pass those up all day long since I know that they are for someone else
